Although the beginning of the new school year is still months away, the Galt Joint Union Elementary School District will begin accepting registrations Monday for the 2011-12 school year.
Registration forms can be picked up at any school office or can be downloaded at www.galt.k12.ca.us. Parents are urged to register their children early to assist the district in planning for next school year. Children are not required to be present at the time of registration.
Enrollment in kindergarten requires that a child be 4 years and 9 months of age by Sept. 1, 2011. The birth date must have been on, or before, Dec. 2, 2006.
Enrollment in first grade requires that a child must be 5 years and 9 months of age by Sept. 1, 2011, with a birth date on, or before, Dec. 2, 2005. Children currently in kindergarten now are automatically registered for next year’s first grade.
The following documents are required:
- Birth certificate (or copy), notification of birth registration, passport, baptismal certificate or affidavit (forms available at any school office).
- Written record of immunizations for polio, measles, rubella, mumps, diphtheria, whooping cough, tetanus, varicela and hepatitis B series. Religious or medical exemptions are acceptable. A medical exemption requires written verification from a physician. Parent/guardian’s signature is required on the personal belief affidavit for religious exemptions. Students who have not completed their immunization series will be considered a “conditional admittance” and must have all immunization records completed by April 14 or be removed from class lists. A physical exam is required for first grade only. Forms are available at any school office.
- Oral Health Assessment is required in either kindergarten or first grade — whichever is the student’s first year in public school. The assessment must be within the 12-month period before your child enters school and must be performed by a licensed dentist or other licensed or registered dental health professional. Galt Elementary can refer to local dentists who can perform the required oral health assessment at no cost. Further information on this requirement will be provided upon registration.
- Proof of residency, such as water, electric or gas bill.
